Medical Records Transfer
  • You may request transfer of your medical records to another clinic at any time.
  • A signed Medical Consent Transfer Form is required.
  • An administration fee of $20 applies.
My Health Record

My Health Record is Australia’s secure, national electronic health record system that allows you and your healthcare providers to access a summary of your key health information online—anytime and anywhere, including in emergencies. It can include immunisation history, medications, allergies, pathology and imaging results, prescription records, and hospital discharge summaries. You can view and manage your record via your ‘myGov’ account or the ‘my health’ mobile app. Our doctors and nurses are happy to update your My Health Record during your consultation, helping ensure your information is accurate and kept up to date.

Home Visits

Some of our doctors provide home visits in certain circumstances, usually for our regular, long-term patients. Because factors such as distance, medical needs, and individual situations are considered, home visits are offered at the doctor’s discretion. If you believe a home visit may be appropriate for you or a family member, please contact our reception team to discuss your needs.

AI Scribe

At Andrew Place Clinic, some doctors use a secure AI scribe to assist with clinical documentation. With your consent, the AI scribe can transcribe and summarise parts of your visit directly into your medical record. All information is handled under strict privacy and security standards, and your clinician reviews and approves all notes before they are saved. If you have any questions or concerns about the AI scribe, please speak with your doctor or contact our reception team.
